KNK Recruiting, LLC is looking for a direct hire Manager of Ambulatory Care for our targeted client located in the Gallipolis, Ohioarea.
Targeted Base Salary: $64,000yr- $81,000yr - Salary varies depending upon skills and experience level.
Client Description: Ourtargetedclient is a a multi-discipline healthcare system, providing services in over 30 areas of expertise, with over 160 providers administering care in 21 locations throughout southeastern Ohio and western West Virginia.
Summary: Under the general supervision of the Director of Population Health, the Manager of Ambulatory Care Coordination serves in an expanded nursing role to coordinate and provide supervision, health/disease education and clinical coordination of the care managers and care coordinators.
Essential Duties & Responsibilities:
Collaborate with payers, primary care providers, site managers and directors at multiple practice sites.
Facilitate clinical guideline adherence and coordinate services specifically related to the patients receiving care management services, including transitional and longitudinal care.
Responsible for integrating identified workflow and care coordination pathways
Perform clinical documentation guidelines to ensure that patients are receiving appropriate ambulatory care management services.
Qualifications:
Registered Nurse with current license required
BSN or comparable Bachelor's degree preferred.
Minimum of three years of management experience or recent experience to match responsibilities, such as acute care, home health or skilled nursing facility required.
Critical thinking skills and ability to analyze complex data sets required. General computer knowledge and capability to use computers required.
Experience as a Health/Disease or Case Manager in PCMH preferred.
Experience with IT solutions such as electronic health record, email, Microsoft software such as Word and Excel and/or care management systems.
